What is Geriatric Care Management?
A geriatric care manager is a professional who specializes in assisting older people and their families with long-term care arrangements. Most have advanced degrees in gerontology, social work or nursing.
Geriatric care management includes evaluating the needs of the client, determining progress and prescribing a course of action. This often involves the geriatric care manager acting as a liaison between the family members, physician, attorney, financial advisor or anyone else involved with their clients’ welfare.
